Your Rights and Protections Against Surprise Medical Bills
(Effective January 1, 2022 — per the No Surprises Act)
At I-MAZE I INC, we believe in helping you make informed healthcare decisions. Federal law protects you from surprise medical bills when you receive certain kinds of care. Below is a summary of your rights and how they apply to services accessed through our platform.
🛡️ What is “Surprise Billing” or “Balance Billing”?
When you receive medical services, you may owe out-of-pocket costs like a copay, coinsurance, or deductible. If you see a provider or use a facility that’s not in your health plan’s network, you may be charged more — or the full amount — unless protections apply.
This is called balance billing, and when it happens unexpectedly, it’s referred to as a surprise bill.
🏥 You Are Protected From Surprise Billing When:
✅ You receive emergency care, even at an out-of-network facility
✅ You receive non-emergency services at an in-network hospital or ambulatory surgical center, but are seen by an out-of-network provider (like an anesthesiologist or radiologist)
✅ You receive care through a health plan that includes federal protections (most commercial plans)
❌ Out-of-network providers and facilities cannot:
- Charge you more than your in-network cost-sharing amount for covered services
 - Send you a balance bill for emergency care, even if provided by an out-of-network provider
 - Require you to waive your rights to avoid extra charges
 
🧾 Good Faith Estimate (GFE) for Uninsured or Self-Pay Patients
If you do not have insurance or do not plan to use it, you have the right to receive a Good Faith Estimate explaining how much your medical care is expected to cost.
- You must receive your estimate in writing at least 1 business day before your scheduled service.
 - If your final bill is $400 or more above the estimate, you can dispute it.

I-MAZE I INC connects users with licensed healthcare providers and pharmacies who may bill insurance directly, including Medicare and Medicaid. While I-MAZE I INC is not a medical provider, we support transparency by:
- Ensuring providers do not bill you directly if you are not insurance-eligible
 - Recommending you verify with your pharmacy whether any out-of-pocket costs apply if your coverage is denied
 - Providing access to federally protected billing rights, regardless of the provider’s network status
