Please read these Terms and Conditions (“Terms”) carefully before using our website, services, or engaging in any telehealth consultation or durable medical equipment (DME) request facilitated by I-MAZE I INC (“Company,” “we,” “us,” or “our”). By accessing or using our website or services, you agree to be bound by these Terms and our Privacy Policy. If you do not agree, please do not access or use our platform.


1. Technology Platform Disclaimer

I-MAZE I INC is a technology and administrative services company that connects users with independent, licensed healthcare providers and pharmacies. We do not provide medical care, pharmacy services, or durable medical equipment directly. All medical and pharmacy services are rendered by third-party licensed professionals and suppliers.

By using our platform, you acknowledge that:

  • You may be matched with a licensed healthcare provider for a telehealth consultation, based on your location and insurance status (including Medicare or Medicaid).
  • You may also be referred to a pharmacy that can fulfill prescriptions and may bill your insurance directly.
  • I-MAZE I INC does not influence medical judgment, determine eligibility, or guarantee service availability.

2. Medicare/Medicaid Billing Disclaimer

The providers you may be connected with through our platform may be enrolled in Medicare, Medicaid, or other government-sponsored insurance programs. However:

  • The providers, not I-MAZE I INC, will determine whether you are eligible for coverage and will bill your insurance directly if applicable.
  • If you are not insurance-eligible or if your coverage is denied, you will not be billed by the provider for services arranged through this platform.
  • If you are a dual-eligible beneficiary (enrolled in both Medicare and Medicaid), providers cannot and will not bill you for any Medicare cost-sharing, such as deductibles or copayments, in accordance with federal law.

3. No Surprises Act Disclosure

Under federal law, you have protections against surprise billing and balance billing when receiving certain types of medical care.

You are protected from surprise bills when:

  • You receive emergency care, even if out-of-network
  • You are treated by an out-of-network provider at an in-network facility (e.g., hospital or ambulatory surgery center)

What does this mean for you?

  • You are not required to pay more than your in-network cost-sharing amount for covered services in these situations.
  • You cannot be balance-billed for the difference between the provider’s charge and what your insurance covers.
  • You also have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency care if you are uninsured or self-pay.

ℹ️ How This Applies to I-MAZE I INC Services

I-MAZE I INC connects users with licensed healthcare providers and pharmacies who may bill insurance directly, including Medicare and Medicaid. While I-MAZE I INC is not a medical provider, we support transparency by:

  • Ensuring providers do not bill you directly if you are not insurance-eligible
  • Recommending you verify with your pharmacy whether any out-of-pocket costs apply if your coverage is denied
  • Providing access to federally protected billing rights, regardless of the provider’s network status

4. Durable Medical Equipment (DME) Disclaimer

If a provider prescribes durable medical equipment (DME) as part of your care:

  • I-MAZE I INC does not supply or deliver DME directly.
  • DME is fulfilled by third-party suppliers or pharmacies that are responsible for billing your insurance.
  • Coverage is not guaranteed and may vary depending on your insurance plan, state, and medical necessity.
  • It is your responsibility to verify benefits and costs directly with your insurer.

5. Pharmacy and Prescription Disclaimer

Healthcare providers on our platform may refer you to a pharmacy to fulfill any prescription. You acknowledge that:

  • You have the right to choose any pharmacy you prefer to fill your prescription.
  • Some providers may have a relationship with a specific pharmacy for convenience, but this will be disclosed where required by law.
  • I-MAZE I INC does not own or control any pharmacy and does not receive referral fees or compensation for pharmacy use.

If your prescription is sent to a pharmacy:

  • The pharmacy may bill your insurance directly.
  • If you are not approved for insurance coverage, you may be responsible for costs. We recommend contacting the pharmacy directly to inquire about any potential out-of-pocket charges before filling your prescription.

6. User Responsibilities

You agree that:

  • You are at least 18 years old or the legal age of majority in your state.
  • You will provide accurate, current, and complete information to us and the healthcare providers.
  • You will not misrepresent your state of residence, health condition, or insurance status.
  • You understand telehealth consultations are not a substitute for in-person or emergency care.
  • You will use the platform for lawful, personal healthcare-related purposes only.

7. Limitation of Liability

To the fullest extent permitted by law, I-MAZE I INC disclaims liability for:

  • Medical care, treatment outcomes, or decisions made by third-party providers.
  • Insurance eligibility decisions, denials, or billing disputes.
  • Errors or delays in the delivery of prescriptions or DME by third-party partners.
  • Any indirect, incidental, or consequential damages from use of our platform or services.

8. Intellectual Property

All content, trademarks, and functionality on this website are the intellectual property of I-MAZE I INC. Unauthorized reproduction or use is prohibited.


9. Modifications

We may modify these Terms at any time. Updates will be posted on this page with a new effective date. Continued use of the platform constitutes your agreement to the revised Terms.


10. Governing Law

These Terms are governed by the laws of the State of Florida. Any disputes shall be resolved exclusively in the courts of Palm Beach County, Florida.
